Replace an Item Form

If you have lost AOK Library materials and would like to purchase replacements, please contact us using the form below. Your request will be reviewed within 7-10 working days. You must have approval to purchase replacement materials and will still be responsible for fines/service fees. Call the Circulation Department (410-455-2354) and ask for a staff member if your lost materials are preventing you from graduating or class registration.
